Oak Valley Health is seeking a Registered Practical Nurse (RPN) for a part-time temporary role at the Reactivation Care Centre in North York, Ontario. The RPN will be an integral part of an interprofessional team, focusing on patient activation, professional practice, and satisfaction. Responsibilities include comprehensive assessment, care planning, patient education, and discharge planning to ensure continuity of care. The role requires a current Certificate of Registration with the College of Nurses, BLS certification, and a minimum of 2-3 years of related experience in a medical or long-term care environment. Experience in palliative care and IV therapy is an asset.

What we are looking for:
As a Registered Practical Nurse you will be an integral member of the interprofessional team at the Reactivation Care Centre. You will practice in an environment where the emphasis is on activation, professional practice and customer satisfaction. Using your clinical expertise, you will, in collaboration with the team, be responsible for comprehensive assessment, care planning, patient education and discharge planning to support the continuity of care throughout the patient's stay on the inpatient unit.
