Daymark Health is seeking a Care Team Manager in Providence, RI to lead daily operations and manage a team including RNs, NPs, LICSWs, and Health Partners. This role involves overseeing schedules, developing policies, ensuring smooth care delivery, and building a high-performance team. The manager will use data to monitor performance, drive continuous improvement, and manage relationships with local providers and stakeholders. A strong background in patient care delivery, clinical operations, and people management is required, with a preference for a clinical background (RN, NP, LICSW).

WHAT YOU'LL DO
As the Care Team Manager for Rhode Island, you will be responsible for leading the daily operations of the Care Team, including Registered Nurses (RNs), Nurse Practitioners (NPs), LICSWs, Health Partners, and other staff. You will manage team members, oversee schedules, develop and enforce policies and procedures, and ensure smooth care delivery both virtually and in our patient’s homes.
As a key leader for the Rhode Island market, you will be responsible for building and developing a high performance team, establishing and meeting success metrics for the region, managing relationships with local providers and other key partners, and partnering with the Market Operations Leader and Medical Director on strategic initiatives.

Additional Information
The expected compensation for this role is $120,000 - $150,000 annually.
This role will be in person Monday-Friday from 8:30 AM to 5 PM ET in our Providence, RI office.
